Program Change Forms

Forms are available by CLICKING ON THE SECTION/DOCUMENT NAME of the sections below. If several forms may be required, they are linked within the content of the section as opposed to the section name. All forms relate to required notification or approvals related to the E section of the Standards.

Change in Degree

This form is to be used by programs reporting changes to the degree granted at the program completion. ARC-PA requires notification no less than six months prior to implementation (Standard E1.08a).

 

Change in Program Faculty

This form is to be used by programs to inform the ARC-PA of personnel change. This includes resignation, termination, or appointment of the program director, medical director or program faculty. Changes in program faculty require ARC-PA notification within 30 days of the date if the effective change (Standard E1.05)

 

Change in Program Fiscal Support

This form is to be used by programs to inform the ARC-PA of anticipated decreases in program fiscal support. Anticipated decreases in program fiscal support must be submitted to the ARC-PA six months prior to the anticipated change (Standard E1.08e).

 

Change in Program Length

This form is to be used by programs to inform the ARC-PA of a change in program length. Changes to the program length greater than one month required ARC-PA notification six months prior to implementation (Standard E1.08c).

 

Change in Program Requirements for Program Completion

This form is to be used by programs to inform the ARC-PA of a change in program requirements for program completion. Changes in the requirements for program completion require ARC-PA approval six months in advance of implementation (Standard E1.08b).

 

Change in Program Sponsoring Institution Personnel

This form is to be used by programs reporting changes in the person the Program Director reports to the Chief Administrative Officer.  Program required to submit the form within 30 days of change.

 

Change in Program Sponsorship

This form is be used by programs reporting proposed changes in program sponsorship. Proposed changes in program sponsorship must be submitted to the ARC-PA as soon as the program begins considering such a change (Standard E1.09).

 

Curriculum Vitae Template 

This form is to be used with the Change in PA Program Faculty form when reporting a new program director, medical director, or program faculty or a change in role for any of those positions per Standard E1.05.

 

Exceeding Class Size Explanation

This form is to be used by programs to report information about numbers of PA trainees exceeding the maximum entering class size, as noted in the class enrollments entered by the program in the PA trainee Tab of the program portal.

 

Faculty Temporary Vacancy

This form is to be used by program when there is a temporary vacancy of personnel greater than 30 days in its positions of program director (or interim), medical director (or interim), or program faculty. The notice must include the program’s plan to accommodate the temporary absence (Standard E1.06).
