January 2012: The budget template associated with Standard A1.07 within the application has some errors in it. Programs are asked to use the budget table available here and to submit it with their application materials in Appendix 1. Within the application narrative for A1.07, please refer readers to Appendix 1.
November 2011: Those using the application dates June, 2010 should also complete and append this C3 supplement in Appendix 14.The application dated October, 2011 has been edited for this standard.
The documents on these pages are provided to assist programs in planning for continuing accreditation visits and reporting any proposed program changes to the ARC-PA. Programs should contact the ARC-PA if they have any questions about the forms or materials.
