** Cuyahoga Community College combined its PA and SA programs into one PA program in the Spring of 2005.

**** The Army program was first accredited in June, 1973. It closed in August, 1977 and was accredited again in September 1980 under the name US Army Academy of Health Sciences. Accreditation continued under the name of Army Medical Center and School AMEDDC&S (Academy of Health Sciences). In May, 1996 the ARC-PA accepted the transfer of the program's accreditation to what is now called the Interservice Physician Assistant Program Academy of Health Sciences.
